TRC (Transitions of Care: Medication Reconciliation)   

This month’s Hints for HEDIS will cover best practice tips for medication reconciliation post-discharge. 

Measure overview: This measure includes members age 18 and older who were/will be discharged from acute or non-acute inpatient care between 1 de janeiro and 1 de dezembro, 2022 and who have their discharge medication list reconciled within 30 days of discharge.

The denominator for this measure is based on discharges, not members. If members have more than one discharge, include all discharges on or between 1 de janeiro, 2022 - 1 de dezembro, 2022. 

Exclusions: Members in hospice or using hospice services in 2022.

Helpful reminders:

  • A medication reconciliation performed without the member present meets the criteria. 
  • Medication reconciliation MUST be completed by one of the following qualified staff: prescribing practitioner, clinical pharmacist, registered nurse, or physician assistant

Best practice tips: 

  • Ensure that patients understand how to take their new medications and know which medications they should discontinue.
  • Encourage patients to bring all their prescribed and over-the-counter medications, including supplements and topical medications, with them to their follow-up appointment.
  • The medication list may include medication names only or may include medication names, dosages, and frequency.
