This policy documents payment guidelines for Collaborative Care Management Services (CoCM) and other General Behavioral Health Integration (BHI) Services. Effective 1 de junho, 2019, in addition to BlueCHiP for Medicare, coverage for other BHI models under CPT code 99484 is extended to Commercial products for providers that have submitted a program description and received approval from BCBSRI. Transitional Care Management Services codes  PF  

Effective 1 de maio, 2019 the Transitional Care Management CPT codes 99495 and 99496 will be “covered and separately reimbursed" for Blue CHiP for Medicare members only. Vitamin D Testing codes  PF
