Beginning in 20 de janeiro21, BCBSRI is offering remote patient monitoring to Medicare Advantage patients with either congestive heart failure or chronic obstructive pulmonary disease. The goal of this program is to help members understand their health condition, how to best manage it, and to have a role in maintaining their health. The program serves two important purposes. First, it provides an early warning if a participant is decompensating, enabling a clinician to prioritize timely outreach or coordination of an in-person visit. Second, when biometric readings deteriorate, a clinician can help the participant connect the dots, linking cause and effect. The result will better inform members leading to improved satisfaction and reductions in hospital inpatient admissions, ED visits, and readmissions.
